In 2019, nurse RaDonda Vaught accidentally administered the wrong medication to a patient, resulting in the patient’s death. Vaught was charged with reckless homicide. Were safety protocols in place, sufficient and being practiced? Was this human error worthy of a reckless homicide conviction?
Join us for an analysis of the RaDonda Vaught case using a patient safety lens and applying a just culture framework to human errors. Just culture requires a change in focus from errors and outcomes to system design and management of the behavioral choices of all employees.
